Couse Dates:

• Saturday (9.30am – 4.30pm): 28^th April & 23^rd June
• Monday (7pm – 10pm): April 23^rd, 30^th, May 14^th, 21^st, 28^th, June 18^th, 25^th

Please read through these notes before completing your application

You can either fill in this online ITB application form or download the application form. All answers should be completed in full, regardless of your position within the church or living waters ministry.  Each application is taken on its own merit.

Whether filling in the form online or posting it to us, you must also send us:

• A page containing your healing journey so far (see page 5).
• A cheque or postal order for £15 (sterling only) made payable to ‘Living Waters’ which is a non-refundable sum to cover administration costs whether or not your application is successful.
• One A5 stamped self-addressed envelope.
• A reference from your church leader or person in pastoral position (confirming they know you & your suitability to be in small group where self-disclosure is required).

Applications should be made as soon as possible and can not be considered until the above information and amdministration fee has been recieved.

Sending in an application does not guarantee a place on the course.

The cost of the course is £175. The balance of £160 is payable subject to acceptance. Payment can be made by instalments if needed.
